A nonprofit community health center on Chicago's north side, Heartland Health Centers provides primary healthcare, urgent care for oral health, case management, and assistance with applying for benefits and entitlements for people who are experiencing homelessness. This program also provides primary care services at outreach sites where people experiencing homelessness stay throughout the Chicago metropolitan area. Assignments with Heartland Health Centers usually highlighted the organization's commitment to serving Chicagoans most in need.
